Need the web site for the class action suit for credit card conversion fees international to file claim?

I need the web site to file a claim where when you used your credit or debit international a conversion fee was charged. I think the time frame was from 1998 to 2005. this is a classaction suit for customer who used cards international. I assume a fund was set up for these claims.

   I need the same site…having received a large form needing my filling out before leaving the country a month ago, I can no longer relocate the form…surely an online search can assist though!
